What To Expect from Home Rehabilitation
- A therapist will call to schedule the first visit.
- During that visit, you and your family member(s) will be interviewed and evaluated for your medical history, previous activity level, medications, goals and expectations followed by a physical assessment. This may include your ability to:
- move and use your arms and legs
- get in and out of beds and chairs
- walk
- do your self-care activities
- bathing, getting to the bathroom, using the telephone, preparing meals, eating and swallowing.
- see, hear, speak and how well you sense things in your hands and feet
- maintain your balance
- think, remember and plan
- tolerate physical activity (your heart, lungs and muscles)
- The therapist(s) will review everything with you and discuss your goals and the plan of care.
- Together, you will develop a schedule of visits.
- The therapist(s) will write a report that includes these goals and treatment plans, and will give a copy to your doctor and the nurse for his/her input and approval.
- The therapist(s) will come to your home and work with you on your treatment program. They will communicate regularly with each other and the other professionals involved in your care. You'll probably be given exercises to do between treatment sessions. Your program will be adjusted and visit schedules may change as your condition changes.
- Once your goals are met, you will be discharged from Home Care and will probably be referred to an outpatient rehabilitation clinic or other resources near you. If you no longer need rehabilitation, then you will be discharged from home care and asked to continue to do a regular series of exercises.
How to Access Home Health Services
The doctor will order home care services when you or your family member is discharged from the hospital or a nursing home. Sometimes, the doctor may recommend home care during your outpatient office visit. You or your family member can always request home care services directly to the Home Health Agency. The agency will then contact the patient's physician to coordinate care. If you contact SPECTRUM Therapy, we can recommend an agency for you. Choosing an Agency
You and your family have a choice in which home care agency to choose. When determining which agency or service to use it is important to ask the following questions:
- Will they communicate with my doctor to coordinate care?
- Do they have adequate staff and experience to provide the intensity of care I need?
- Do they have specialists or experts in my area of need?
- What is the typical visit frequency and length of treatment for someone with a problem like mine?
- What are the agency's certifications and accreditations (JCAHO and/or CHAP)?
- Is the agency affiliated with a Medical Center?
- Can the agency provide all the services I may need such as; registered nurses, home care aides, therapists, pharmacists, psychiatric services, nutritional support, infusion therapy, social workers, and medical supplies?
- Does the agency participate with my insurance company?
- Is the agency licensed by the Virginia Department of Health?
- Is the agency Medicare certified?
- How does the agency select and train its employees?
- Does the agency conduct a criminal records search for each employee?
- What procedures are in place to handle emergencies?
